Welcome to the enrollment system for Samueli Academy!  Once you have completed this process you will be able to print the registration packet including any pages that will require your signatures.  The process should take 30-45 minutes.

You will need certain information to complete this process including:

• an e-mail address
• basic personal information
• primary physician information
• immunization records (required immunizations listed HERE)
• contact information for previous school attended
• information for at least one emergency contact

ALL of the following documents must be received with this completed packet by its due date:

• THIS PACKET printed and signed. (You can print it at the end of the process.)
• Copy of transcripts, current schedule, and current grades (current school will provide upon request).  **This information is essential in order to start planning for fall semester.**
• Current Immunization Record
• Birth Certificate
• Proof of Address (example: current utility or cell phone bill)
• If applicable: Copy of IEP, 504 Plan, restraining order documents, or child custody documents, social worker info, and physical evaluation form, if your student is interested in playing sports next year.
